Create custom input form for OMALS submissions

Keith Gilbertson requested to merge omals_submission into vt_7_x_dev

Duplicates work originally done by Anne and Kimberli for OMALS submission form in DSpace 6. This update for DSpace 7 was done manually because we had some problems with the automatic conversion tools.
